add javascript

We have a salesforce web to case HTML form. This form has dependency fields that should only show when a certain choice is made from the parent field. According to Salesforce, we need to add our own javascript to make the child fields behave how we want. The fields are already assigned as dependency fields however making the Web-to-Case HTML results in the dependency not work.
