Write javascript code on add field on form (metronic framework)

Hello everyone , Im using metronic framework : https://themeforest.net/item/metronic-responsive-admin-dashboard-template/4021469
Now , i want to add layout look like my attach file with javascript code , mean that when i click "+" , it will add some fields automatically. If i click "-" , it will remove that all fields.
Provider and Services are relationship "parent -child"
Result : when i click save button : all information from layout , it will push to api services.
Anyone has javascript experiences can do that ? ( I will all thing you need - please ask me )
