User Feedback Form in JavaScript

Looking for freelancer to create Complain and Suggestion Form using HTML and Vanilla Javascript with folloing HTML pages:
1. Landing or Splash Screen with Animation.
2. Login page with Username and Password,
3. Complain Page as per standard information(get it form internet or use your experience),
4. Suggestion Form same as Complain Form.
5. Dashboard with Visual to show analytics for both Complains and Suggestions.
The primary purpose of this form is to enhance user experience by resolving service-related complaints, thereby improving our service delivery.
Your assignment will be to set up a variety of input methods including, but not limited to:
- Text Input field
- Dropdown Menu
- Date field
- Multiple Selection Dropdown
- Checkbox
The ideal freelancer for this project should have a strong background in building responsive designs using HTML and Vanilla JavaScript. A solid understanding of user experience is also crucial, as the form needs to be user-friendly and facilitate effective communication between us and our users.
