Survey Web Page with backend

A simple survey web page is needed, where the access of the person who is going to do it is controlled with an ID number authentication, after that, it is stored in a database. With the survey responses, a custom PDF must be created for each survey and stored in a storage database. As a Frontend the survey must show a Simple Web Page, as backends the results of the surveys must be displayed with cakes and graphics, also with the backend you must have access to the stored pdfs with a search engine and search filters. Firebase can be used for authentication, database, storage, hosting and domain. Firebase is not required, you can use another database. Take into account that at least 30 thousand PDFs will be created.
