I need a simple web form that connect a database to track status of vacinnes in a company.
Each time people get in the company we need to check their vaccines and health status with a simple form.
We need track and report incidents like people with simptoms, track their quarentine, track status of vacinnes...etc
They have to get a report in PDF and a QR code that will be read by security personal (with status of access granted or denied)
Integration with Whatsapp to notify specific people its desirable
Its a simple proyect, max budget 200usd
