Simple React JS Reusable Form Components

I'm working on a class project and need a set of simple, reusable and extendable React JS form components.

Specific Requirements:

- Form Components: Primarily input fields, dropdown menus, and date/date range fields. The data can come from mocked data/sample set. This is a rule based system. Each rule can have multiple items like a drop down and an input field.

Something like
Example 1:

- Styling: Basic/Minimal design approach. No complex or heavy design elements, just simple and clean styling. Basic text styling not important.

Ideal Skills:
- Proficiency in React JS

- Familiarity with basic/minimal design principles
- Ability to implement simple, clean and functional designs.
