Simple Page for MD5 Hashing

I need a single page web application that contains 5 text boxes. Once text or a number is inserted into any of these boxes, the page should automatically compute and display the MD5 hashed value of the input below the respective text box. The text boxes themselves should allow for user input, and the hashing should be immediate and visible.
