Simple Flask app for AI model

I have 2 trained models. Need to make a simple web app using jupyter notebook code and deploy.
>Input is an excel file.(main page)
>Page should also have an option to manually enter values(enter values button on class click loads form with inputs).
>On submit do the action of running backend code in first model jupyter notebook.
>Code output is a dataframe to be displayed and button to download as excel.(success page)
>Second model is to predict and plot image. Image needs to be downloadable.(plot button on success page redirects to this plot page. Need to have a main page redirect button)
> Deploy app on any platform.(gitlab preferred)
