Simple DocuSign API Integration

I'm looking for a skilled developer to implement a straightforward DocuSign API integration within a two-day timeframe. I need simple HTML form that collects a few fields of data. The information from this form should populate 2-3 fields across two PDFs which will be sent via the DocuSign API for signing by two designated recipients.

- HTML form: Collects basic information from users
- DocuSign API: Populates data from the HTML form into 2-3 fields across two PDFs and sends them for signing.
- Viewing Page: A simple page that displays the HTML form data alongside the signed PDFs in a table view.
