Shopify Developer for “One-Time Free Consultation” Flow (OTP, Metafields, Custom Liquid)

We are a US-based supplement brand. We sell on Amazon and Shopify and want to offer a one-time free consultation (Nutrition and/or Personal Training) to our customers.
The key tasks involve:
1 - Implementing Shopify’s new account system (OTP login).
2 - Building a custom sign-up form that stores data in Customer Metafields.
3 - Blocking repeat consultations (one per user) and differentiating auto-approval for Shopify orders vs. manual approval for Amazon orders.
4 - Sending a “Thank you” email to the customer and an internal notification to our team.

Application Requirements:
1 - Relevant Shopify Customization Examples:
1a - Share at least one example of a custom Shopify theme or Liquid customization you’ve done that relates to forms, order validation, or metafields.
1b - Include a short Liquid code snippet (or a link to a GitHub gist/repo) demonstrating your custom logic.
1c - We will not consider proposals lacking code samples or relevant Shopify portfolio links.
2 - Willo Pre-Screen (Mandatory):
2a - After applying here, complete a short Willo video pre-screen (link below).
2b - We’ll ask about your approach, timeline, and for you to explain the Liquid snippet you provided.
2c - Only candidates who both share a Liquid code sample and complete the Willo interview will be considered.
