Senior VisualBasic.NET Developer need

The application will be used for scheduling service agents to customers. We will need an interface for call center employees to enter the customer's information and required work to be completed. The interfaces will need to be client-side code heavy (Javascript) using AJAX. Info will need to be stored in a Mysql database. We will also need an interface that will allow us to schedule and track the lifecycle of the service. In your response please mention the window title of the example schedule so I can see that you actually r eviewed the project. I have attached some example below of what we are looking for with interfaces. This is only a small portion of a much bigger project. Great performance will be rewarded with additional projects.
