Script for Webpage for my JavaScript/Button

I need to give a script to a website company for adding my javascript/button in their page. Currently I know what changes they should do but they need a script. They gave me an example how other site has put two buttons in their site by giving them a script.

Issue is we need two variables from website company too. Let's see if you can help.
