React + Typescript + storybook

I have attached 2 pdf of the mockup for the web page. I need it to be coded using react, typescript and storybook. Need to use Material UI and material icon for latest react version 4.

Makes use to capture the input field, log the response in the console. You can use the dummy data to get the field populated.
Please use typescript.
Make sure all the drop-down populated something, even if it is just one option. I want to make all the click functionality works, what I meant is when the form field is field some kind of dummy data populated the page.
Please break into smaller components where possible.
