Persoanlity Quiz -- 2

This is for a family friend trying to launch a website. They are communications professors and have developed their own leadership method/course. They are looking to make an 18 question form that gives you a score based on a personality acronym that has been developed. The Personality test is broken down by the acronym AGENCY. Each letter has 3 questions associated with it. The questions are asked in the form of Never, Rarely, Sometimes, often, and always. It would also be great if the questions could be in randomized order after each test. We want the results to be on a point line graph with the letters of AGENCY on the x-axis and the score on the y-axis. It would also be great if the graph was highlighted into regions of green, yellow, red to indicate the strength of score and areas that need work. We would also like the last couple of questions to be demographic-based and also a section for emails. This will be embedded into a square space website.
