Online Form

I am looking for a freelancer who can create an online form for gathering contact onboarding information using either Adobe, Typeform or something user friendly and dynamic. The form should have more than 10 fields. I do not need any specific software integration, but I would like the form to be convertable to Excel or csv format and be able to pull through attachments. The form should also be customisable by the user to enable sections they need to be displayed and sections not chosen on the first page to be hidden. First page will use their answers to tailor the form.

Ideal skills and experience for this job include:
- Proficiency in web development languages such as HTML, CSS, and JavaScript
- Experience in creating online forms and data collection systems
- Knowledge of database management and data storage
- Ability to ensure data security and privacy
- Familiarity with file handling and attachment management
