Okta custom login javascript widget

For anyone familiar with the Okta javascript client side login widget, we need the standard javascript widget modified to either pass additional information as extra fields in the sign in form to okta application, or change application ids depending on the form field. Probably will require a customization of the current, but locally stored, okta javascript library. Note: this is the 'okta sign on widget' with client side javascript, not the server side / npm type application.

We also need to add a customized link added to the form widget to okta to register/signup.
