Need a script to require fields below a checked box in a form (but not required if it is not checked)

I have a form with checkboxes that also have three fields below each of those checkboxes. I need those fields to be required ONLY if the preceding checkbox is checked - but if the checkbox is not checked, leave the fields not required.

I currently have this working for selects in the form, but I need the same logic applied to checkboxes - see uploaded screenshot.

This is a JavaScript/HTML requirement.

If you can't get to this today, I really can't use your help. For you expert developers, this is likely a 10 minute fix - if that long.
