Need 2 screens React PWA

I need a React Progressive Web App (PWA) with two screens that will be used on mobile devices. The purpose of the screens is to collect user input through a form submission. Ideal skills and experience for this project include proficiency in React and PWA development. App is very simple with all data saved locally without any API integration.

I also need a daily push notification using onesignal and google analytics integration.
