Multi step form
Budget: $30 – $250 AUD
I need to create a multi step form using Bootstrap. I have included the form steps and the sections/fields within each step of the form.
The form must be created using bootstrap where possible.
The form must only use standard jquery if required, ideally just vanilla javascript.
I have attached an excel document summarising all of the fields required, I have also attached a rough example of what the form should be styled as.
PLEASE NOTE: This form will be embedded with an existing bootstrap web page so ideally it is all contained within a single HTML file. Jquery and bootstrap are already imported to the parent web page through CDN.
