Knockout modal popuup

I have a knockout js and html form wherein I have a name, telephone, dob field when we press submit. if dob entered is less that age 16, then a modal popup appears where it asks for parental consent as attached, and there are two buttons no thanks and ok, if they press ok, then a popup appears with a pdf loaded with ok button, then if we press ok the submit in main form will be enabled
