Javascript Credit/Debit Payment Form

We need someone to create a javascript payment form using the instruction from here -> https://uat.dskbank.bg/sandbox/en/integration/sdk/web_sdk_multiframe.html#web-sdk-init
The form does not need to store credentials. We need just the working form using the DSK test library (<script src="https://uat.dskbank.bg/payment/modules/multiframe/main.js"></script>) and will continue the integration from there by ourself.
