HTML/ js personality test

I need to do a “Economic Personality test” similar to this one https://bit.ly/3pIBhpU in HTML/JS script
The user will have to check words in a table of 78 words that I will send you. Each word will add a point to a category (there will be 8 categories). The check boxes will have to have this effect https://codepen.io/dylanraga/pen/Qwqbab/ and also the words will have the same hover effect from there. After the user check all the desire words he will click the button “See results”, after that he will see a text and a form that will ask him his name and contact details (Name, e-mail, Phone, city, country, etc).
I need this in one day.
After he fills the form, he will see another text and will receive in his e-mail the results of the text.
The e-mail with results will have to be an HTML e-mail that will include a table with results for each of the 8 categories. The results for each category are calculated in percentage of the total words of that category (some words are in 2 categories)and how many words of it he had check, Ex. If in category red there are 8 words and he checked 4, he will get 50% in that category. It also will do a graph of the percentage result and if the category is active o passive (if any category has more than 5 words checked, it is an active category if not a passive one). The category lists has to be ordered by the percentage number, the ones that has more percentage has to be on top and the ones that has less percentage under, also it will have to draw a line under the list of the active categories, this line will separate the active and passive categories.

Also, it will have to send an e-mail to the administrator, with a different text, the details of the user (Name, e-mail, Phone, city, country, etc) and it will have also to attach a CSV file exactly like this one http://bit.ly/3JLiEs6 where it will include the details of the user, the results in percentage for each category, the result for each category if it is active or passive and the result for each word, putting 0 if the word was not check and 1 if it was check.
You will have to do this on my server, I will send you CPANEL credentials.
