I want a HTML PAGE, Angular based that collects data from users. (you wont be connecting to the database ill do that, but you will create a form for collecting the data)
The page is single page with multiple layers where client can click next, and can see the progress and position of their application.
I have put two files,
1 is a word document on the fields i want to be put in the form,
2 is an image of how i want to the form to look like.
