A Google Web App - Dynamic Order Form

Optimal interactive dynamic order form for a small business
1. Google Web App i.e. G Scripts, JS, G Sheets, HTML, CSS
2. Form will be pulling and pushing data from and to Google sheets
3. Dynamic build of HTML objects e.g. # of div/option/select,etc will be dependent from other objects which are being updated by users (do not hardcode)
4. With calendar for same-day and advance orders based on Google sheets (with several dependents)
5. It would be a lengthy form in some instances thus the need of accordion or alike design
6. Parts of the order form:
a. Calendar for booking delivery date and time
b. Product to choose and quantity based on inventory per day (present day or future days)
c. Several sub-products to choose based on item letter b
d. Accepts additional order (loop for b and c) max counter is based from active product list in Google sheets
e. Accepts customer info: complete name of the user, recipient name (if user is not the recipient e.g. a gift/present) contact # of the recipient, delivery address, pin location in G maps (if any)
f. Simple captcha
g. Generates invoice that will be shown at the end of the form (with incremental invoice #, product info, discount/promo, regular price, and other vital info for delivery, etc based on G sheets)
