Fedex API label creator form

I need an HTML/Javascript/ CSS static form that Does the following.

Gets user input of the delivery details, creates and prints label.

One page Form
Captures:

- Recipient First Name
- Recipient First Name
Recipient Mobile
- Delivery Address (with getaddress.io autofill address finder)
- Weight
- Dimensions

On pressing next a FedEx shipment will be created, label generated and a button that says print label.
