Extension not being loaded into the headful browser when puppeteer initializes the browser

I need someone with JS puppeteer experience (headful browser using extension) who can help us fix a problem that is causing the extension not to be loaded/run in puppeteer.

With any luck it should be a quick job jumping in a meeting and helping to find the problem in our deployment or puppeteer Nod JS code.
