Custom Reporting - from Google Forms responses

Hello,
We have a google form which is filled by the employees every day for:
- Login (time) and attendance
- Tasks (select project, a few more questions)
- Logout
The format for the response sheet is attached to the project. I am looking for someone who can:
- Create custom form where name of projects and employees can be pulled from another google sheet/ row
- Have option for employees to Log in (using google email account)
- Provide details for the tasks/ projects they have worked on
- Log Out
Regular Google form might not work as we need a little customisation so better if someone can create the form options using scripts.
Please provide details of your similar work experience for us to shortlist and discuss the project further with you. Generic bids will be ignored by default.
