Create a jquery/html form

I need a form with a list of items with edit/delete functionality. The items should be listed in a ul/li and should look likes a list of items for example

1. NAME, AGE, Address - Edit - Delete
2. NAME, AGE, Address - Edit - Delete
3. NAME, AGE, Address - Edit - Delete
3. NAME, AGE, Address - Edit - Delete

If the user clicks to edit the edit button should become save and the data should be editable. Once the user clicks the save button the data should be saved and become (look likes) a list.

If the user clicks to delete the li row should be deleted

The main thing I am looking for is to make the textbox list look similar to a ul/li list. Similar to the attached image.

Needs to get it done quickly.
