Create an Add-In for the desktop Outlook client -- 2

I need an Outlook add-in created that can be deployed as an Office 365 Admin Managed add-in to the Outlook client. It needs to allow the end user to send the selected email from their desktop Outlook client as an attachment to a pre-configured internal email address. It should also give the user the ability to add comments which will show in the body of the email that is sent.
