Convert a Typeform form, into a html form

I am looking for a developer who can convert my Typeform form into an HTML form that is mobile-responsive. The conversion should be basic with no specific customization required. Additionally, I need specific integrations and functionalities to be included in the HTML form making it able to to insert in the text fields only values of a certain column of a table of my MySQL DB.
