Contact Form for WordPress with Repeater Field
Budget: £100 – £700 GBP
I would like the form as can be seen at the top of the following website with the same functionality except using the following API for the To and From fields (https://developers.amadeus.com/blog/django-jquery-ajax-airport-search-autocomplete)

To, From, Date/Time, Passengers (Add Flight/Remove Flight Repeater Field) >> Next Step >> Name, Phone, Email, Additional Info
Happy to use a plugin of your choice as a basis but please mention which one in your proposal so that I can get the licence cost.
This needs to be compatible with Google Conversion Tracking and of course, send emails when the form is submitted to 3 email addresses.
