Collect elementor form and list out the submissions to a page

I have a custom made javascript form to calculate basic loan requirements with only 6 fields. The loan eligibility will be shown on screen. I need to store the submissions into database and then list out the submissions to a static page. Also add 4 more fields to the form(name, email, phone). Budget $10.
