Clinical Chart to be built with React, SVG, D3

Our business develops advanced clinical systems for management of critically ill patients. We have a requirement for a "read only" clinical chart. It will be integrated in another application to be used in acute and sub-acute care. The chart must be built from primitives and not depend on other charting libraries. The chart will need to resize appropriately for desktop screens and connect to a variety of services via an API. It does not need to be editable, but must update data points in real time without refreshing the entire view. I have attached an image of the style we are looking for although the design will be a little different. Please note it is important that the chart code is complete in itself and does not include other charting libraries. That is the code needs to "draw" the chart from SVG primitives such as lines and rectangles. The code base will need to be well documented and structured so that it is readily maintainable.
