Build a slack app

I would like someone to build a form in slack app with following features:
1. Multiple dependent dropdown list (For ex: I would like to view a dropdown list with Countries and once a country is selected, I would like to display states that are relevant to the selected country.
2. Displaying/referencing images or information for each option so user can refer before selecting an option. Attached screenshot for reference.
