Biosolids JS program

We would like for someone to build a small interactive program in JS that we could load into our company website. The program would consist of a graphical user interface or API in which the user would choose values for several parameters. The user would then press the "calculate" button and the selected values would be used to calculate results. If the user did not like the results, he/she could "reset" the API and choose different values.

I am enclosing what we think the API would look like and an Excel spreadsheet that includes the back end calculations.
