Adobe PDF Javascript Help

Working on a package builder that autofill information in a variety of forms. We have received updated forms that need to be used for new applicants, when I update the package builder with the newer forms and remove the outdated forms the javascripts stops and does not autofill to the corresponding fields and basically stops working. I have attached multiple copies of the package builder: The original working copy; a copy with the original documents AND the updated forms; and a copy with just the updated forms that stops working. Looking for help, the javascripts only stops when the original documents are deleted from the package builder, I am new to Javascript and am unsure as to how to proceed.
