Adding records using javascript

I'm building a blood pressure recording app for my family to use. The javascript for adding records is not working properly and shows "undefined" for the values on the page that should display them. When going to the records, add record, only 1 of the 4 values shows correctly and that is the DATE field. The other 3, Systolic, Diastolic and Pulse show undefined.
