Adding code to my site. Knockout JS, simple validation
Budget: $25 – $50 USD
I want to add this code:

With two small tweak:
1) Remove the generate button.
2) Instead of a valid number saying "true", either have a button to apear where a user can click to download a pdf, or have the pdf pop up in a new tab.

Basically I need a user to only be able to download a savings card if the have an NPI number, the code on JSfiddle checks for a valid NPI number, so I just need it added to the page on the site and tweaks to be able to download a pdf.
If a user doesn't have a valid NPI number then they'll just be told to contact customer support
I tired installing knockout js to my site, but it wasnt working for me. The site is built on a custom template, built by me a long time ago.
