Add Select and and Slider to form (Material UI, React, Firebase)

Hi I have a small task to add two fields into a existing form (react, material ui core)

the data needs to be send to our firestore db (connection is already done)

Select Options (label: category)
- Category 1
- Category 2
- Category 3
- Category 4

Slider (label: happymeter)
