Java Form Integration

I need a Java Developer to enhance an existing application by adding a form with basic input fields. The rendering is already in place, and the data is currently stored in a flat file.
I have included an image, the input from will be the last one under "Animations".

Requirements:
- Add a form with the following fields:
- Title field
- Description field
- URL field
- Dropdown list (with names and icons)

- Validation:
- Basic validation (e.g., required fields)

Ideal Skills and Experience:
- Proficiency in Java
- Experience with form handling and basic validation
- Familiarity with flat file data storage

This task can be completed in a few hours by an experienced developer. Please, no beginners.
