Convert a paper application into a Java object but preferrably a Vaadin form

I am looking for a freelancer to convert a physical paper form into a Java object, preferably using Vaadin to create a form and create a document using Apache poi.

The minimum I require is that the paper application is converted into Java classes with proper attributes. I will pay extra for writing the UI, creating the unit test and writing the document generator.

I have finished two of the sections which you can use as examples for the rest of the sections. You don't have to follow the form output exactly as longs as more data fits onto the same piece of paper.

Current Format:
- The paper application is in physical form.

Digital Copy:
- Yes, I have a digital copy of the paper application.

Functionality Requirements:
- Each section of the application must be its own class.

Ideal Skills and Experience:
- Strong proficiency in Java programming.
- Experience with Vaadin framework.
- Knowledge of form creation and data mapping.
- Attention to detail and ability to accurately convert paper forms into digital format.
- Good communication skills to understand and implement the specific functionality requirements.

By accepting this project, the freelancer acknowledges and agrees that any code, work, or deliverables produced for this project shall be the exclusive property of the project creator. The freelancer hereby assigns and transfers all rights, title, and interest, including without limitation any copyrights, trademarks, or other intellectual property rights, in and to the work created for this project to the project creator.

Furthermore, the freelancer agrees that any compensation received for this project, whether agreed upon on this platform or otherwise, shall constitute the full and final payment for the work performed. The freelancer acknowledges that no additional compensation, royalties, or benefits shall be claimed or sought in the future for the work performed on this project.

In the event that any provision of this agreement is found to be unenforceable or invalid, all other provisions shall remain in full force and effect. The freelancer agrees that in such a case, the project creator shall be granted an unlimited and exclusive license to all the work created by the freelancer for this project.

This agreement is binding upon and shall inure to the benefit of the parties and their respective successors, assigns, and legal representatives. By accepting this project, the freelancer signifies their agreement and acceptance of these terms.

1. Jurisdiction: Any disputes or legal matters arising from this agreement shall be exclusively governed by and resolved in accordance with the laws of the State of Florida, United States. Both parties agree to submit to the exclusive jurisdiction and venue of the state courts located in Florida for any legal proceedings.

2. Intellectual Property (IP) Ownership: The freelancer hereby assigns and transfers all rights, title, and interest, including without limitation any copyrights, trademarks, or other intellectual property rights, in and to the work created for this project to the project creator, encompassing all forms of intellectual property, including patents, trademarks, and copyrights.

3. Third-Party Components: The freelancer warrants that any third-party libraries, frameworks, or code incorporated in their work are free to use for commercial purposes and do not impose any restrictions on the project creator's ownership or rights over the final deliverables. The freelancer agrees to indemnify and hold the project creator harmless from any claims or liabilities arising from the use of third-party resources.

4. Non-Disclosure and Confidentiality: The freelancer shall treat any sensitive or confidential information provided by the project creator as strictly confidential and shall not disclose or use such information for any purposes other than completing the project. This duty of confidentiality shall survive the termination of the agreement.

5. Indemnification: Both parties agree to indemnify and hold each other harmless from any claims, damages, losses, or liabilities, including legal expenses, arising out of the creation or use of the work produced for this project, to the fullest extent permitted by the laws of the State of Florida.

6. Revisions and Modifications: Any revisions, modifications, or future updates to the work shall be agreed upon by both parties in writing. Ownership, compensation, and rights for such revisions or modifications will be subject to a separate agreement or negotiation between the project creator and the freelancer.
