Quick Translation for a special needs card

My son is special needs (autism) and it is his dream to go to Japan, so I am taking him this month. He occasionally has meltdowns that can be concerning to the people around him, so I carry cards that I can hand out to defuse any spectator involvement. I need someone to translate the cards, so that I can hand them out in Japan when necessary. The front of the card says:
