i need and adroid and ios developer

I want to take my forms and paperwork and add them to an app so everything can be entered via phone or tablet in one place. Approx 8 main Titles with 17 Forms (multi-line) unlimited submissions and lines. When I enter the data in each form I want it auto saved and able to pull up the forms for examination from a third party.
