Develop an App for iOS and Android

I need an iOS and Android app. I have the complete design (wireframe) and I need to get the app developed. The App will have 9 detailed screens. Each screen is a form with a large number of fields (text boxes, drop downs, check-boxes, radio buttons etc). The App should allow users to save and retrieve information from a database.
I need an Experienced App Developer with a proven record in building iOS/android apps from start to finish. Please let me know examples of apps you have developed by providing the app name on the App Store. Please also let me know how long you think it will take you to get it completed (tested and working) on play store and apple store. Also please let me know what language/tools you plan to use for development.
