Design of a simple Iphone app needed

We have a working web-based application. We need one portion of the application to be turned into an iPhone app. The application is hosted on the website JEVZ.com - and we will need the employee section built into the iPhone app. There are around 12-15 functions, and we will provide the designs that would be consistent with the current web-based app. The input of the data to the current system will be provided by my developer. To see the current app, just sign up and add yourself as an employee and see all of the current features. App is free no credit card is required. Screenshots are attached.
