iPad app, user fill in and store data in icloud

2 page ipad app that allows pictures to be taken gps data to be stored and a checklist to be completed with a dropdown list of static fileds. Approximately 10 filed requiring check boxes, and then the ability to store pictures taken and also gps data to be stored and completed form. Need project done in short time frame.
