iOS app about pre-filled form

We are developing a functional module for an iOS app that provides a pre-filled form for customers to complete and sign. To accelerate the project progress and gain more professional frontend design and development support, we would like to outsource part of the work to your team. The implementation should be done using Swift and Storyboard.

This app has already been implemented on the Android platform, so I have screen recordings and detailed specifications for each page. You only need to replicate the exact same pages.
