Coding a simple reminder app

We are a group of doctors looking for an experienced programmer to create a simple reminder app for our patients to use. We would like the app to be developed for iOS but we don't have a preferred programming language for this project, so I leave that part up to your discretion. The app will have recurring reminders for cancer screening. The patient would input their sex and age and the app would send them reminders. For example a reminder every 2 years to have a mammogram to look for breast cancer. There are 10 total reminders we would like and I have attached them in the spreadsheet below. Building notifications into the app would also be beneficial. We may want to add additional features to the app in the future but this is all we want to start.
