iOS Arabic Feedback Form Templates

I use Apple Numbers on iPhone and iPad to collect client impressions and I want a clean, fully Arabic set-up for this purpose. The task is to design one or more feedback forms inside a .numbers workbook, laid out right-to-left and ready for immediate use.

The form should let me enter ratings, optional multiple-choice answers, and free-text comments. Every label, placeholder, and help note appears in modern standard Arabic; there’s no need for an English version. I also want the sheet behind the form to organise the incoming responses so I can filter or create simple charts later.

Deliverables
• A .numbers file containing the Arabic feedback form(s)
• Any light data-validation or drop-down lists needed to keep entries tidy
• Brief inline notes (or a separate page) showing me where to edit choices or texts in the future

I’ll open the file on iOS first, so please build and test it there before handing it over.
