quick books enterprise template design needed

we are a firm that uses quick books. our version is enterprise. we are looking for a new template called (BOL) bill of lading. here is our example of our invoice and the current BOL we use, which is a pre-printed dot matrix form. we need to build this new BOL form to print on our laser printer.
